Prior to your child's scheduled appointment, you will receive a text from our North Florida Surgeons partner Qure4U (telephone# 43498) with a link to a series of required tasks that are designed to be completed at your leisure before you arrive at our office. These tasks are required to be finished before check-in and include answering important Pediatric ENT medical history questions, confirming your demographic information, and signing various NFS financial agreements. If you have any technical difficulties with the link to these tasks, you can call Qure4U Patient Support (844) 961-2226 or email them patient@qure4u.Freshdesk.com M-F 8 AM- 8 PM EST.
At Check-In, we will ask you to provide us with a copy of your Photo ID and Insurance Card to upload into our system. If you only have a digital copy of your insurance card, or did not bring the physical insurance card or Photo ID with you, please let us know at check-in. We will provide you with a text link so that you can upload a digital file or photocopy and send that to us to upload into our system.
Finally, if you are not able to complete the Qure4U tasks prior to your child's appointment, please come to our office at least 20 minutes early for new patients and 10 minutes early for established patients, to give yourself enough time to complete the tasks in the office before your appointment time.
You also have the option to create a North Florida Surgeons Patient Portal Account for your child.
There you can see your child's upcoming Pediatric ENT appointments, see information about your child's past Pediatric ENT visits, send us a message, or to make online payments to your child's NFS billing account. Online payments can also be made without a Patient Portal Account as a Guest.
