Prior to your child's scheduled appointment, you will receive a text from our North Florida Surgeons partner Qure4U (telephone# 43498) with a link to a series of required tasks that are designed to be completed at your leisure before you arrive at our office.  These tasks are required to be finished before check-in and include answering important Pediatric ENT medical history questions, confirming your demographic information, signing various NFS financial agreements, and uploading your insurance card. If you have any technical difficulties with the link to these tasks, you can call Qure4U Patient Support (844) 961-2226 or email them patient@qure4u.Freshdesk.com M-F 8 AM- 8 PM EST.

If you are not able to complete these tasks prior to your child's appointment, please come to our office at least 20 minutes early for new patients and 10 minutes early for established patients, to give yourself enough time to complete the tasks in the office before your appointment time.

You also have the option to create a North Florida Surgeons Patient Portal Account for your child. 

There you can see your child's upcoming Pediatric ENT appointments, see information about your child's past Pediatric ENT visits, send us a message, or to make online payments to your child's NFS billing account. Online payments can also be made without a Patient Portal Account as a Guest.
